All scientists and cruise participants going to sea on SIO RV New Horizon must submit forms. Since 2014, the forms are submitted electronically using the Ship Scheduling User Portal. New users: please setup a user account to login; existing users can login and update their information & forms.
PDF hardcopy forms are no longer submitted to Ships Scheduling or the Chief Scientist.
- Complete the forms at the Ship Scheduling User portal (Link: new for 2014)

PDF forms for faxing or mail/e-mail:
- Physical Ability to Work at Sea (submitted for every cruise)
- Drug, Alcohol & Sexual Harassment Aboard Ship Policy (submitted once per year, good for all SIO vessels)
- Medical History (voluntary) – recommended if you have a condition that requires regular medication or should be known to the Captain & Chief Scientist to insure your safety.
